Mueller Family McDonald's offers optional health insurance coverage to all full-time (30 hours/week or more) employees. Our annual open-enrollment period is September 1-30th and coverage start date is October 1.

If it is outside the enrollment period, but you have a life event such as marriage, divorce, birth of child, etc., you can fill out this form and someone from our office will contact you to assist you in your enrollment.
